CONTRACEPTIVE PILL REVIEW

Please have a recent (last month) Blood Pressure result for submission. This can be done at the Surgery's BP machine.  Just keep the results printout and enter them into the form.

THYROID/THYROXINE REVIEW

Please do this review while in a relaxed environment, as a pulse reading is required
