COVID Vaccine Certificates
 

Please do not contact Chartfield Surgery regarding vaccine certificates. We cannot issue valid proof of vaccination.

Vaccination Certificates

From 17 May 2021, people in England who have been fully vaccinated against COVID-19 can demonstrate their vaccination status for international travel. A full course is currently two doses for the AstraZeneca Vaxevria, Pfizer Comirnaty, and Moderna Spikevax vaccines, or one dose for the Janssen COVID-19 vaccine. You can prove your vaccination status in the following ways:

  • using the NHS App, which you can download from all mobile app stores 

  • calling the NHS 119 COVID-19 helpline and requesting a copy of your vaccination pass

  • through the NHS website 

​

A record of a full course of COVID-19 vaccination received through any of these services will be valid for overseas travel. It will not necessarily be valid for domestic purposes.

If you choose to use the NHS App service, it may take 7-10 days for your identity to be checked and verified. COVID-19 vaccine certificates ordered online or through the 119 service may take 7-10 days to be printed and dispatched.

If you’re a patient at our practice you can now use the NHS App, a simple and secure way to order repeat prescriptions, view your GP medical record and more. Visit the NHS app website for more information.

To use the app, you need to set up an NHS login. If you have already had your ID checked to access GP online services, you can be fast-tracked if you use the mobile number you supplied to us when you registered.

​

If you have not been set up for online services, you will need to send us one form of photo ID so that we can release the codes which you will need to set up the app. Please submit your requests via email and allow up to 5 working days for your request to be processed. 

NHS App Issues

If you experience any difficulties in logging in to the NHS app, please follow the steps below:

  1. Browse to https://www.nhsapp.service.nhs.uk/login

  2. Choose option to 'Continue with NHS login'

  3. Login by entering your email address and password

  4. If asked enter the security code that you received to your device

  5. On the ‘Manage your NHS login’ screen, select the option ‘Delete NHS login’ under the ‘Delete your NHS login’ heading.

  6. Create new NHS login

This error may occur when you have used your NHS login on more than one platform.
